Community-Based Secondary Stroke Prevention
An Evidence-Based Practice
Description
The Community-Based Secondary Stroke Prevention program is an intervention to improve knowledge about stroke, improve self-health monitoring, and maintain behavioral changes for a healthy lifestyle to prevent stroke. Individuals with minor stroke often underestimate the long-term impact on their health; educational interventions can equip those at risk with knowledge and self-care strategies for secondary stroke prevention. This intervention program includes eight, once-weekly group educational sessions to improve participant awareness of health signals. The sessions include lectures, games, and exercises to promote group interaction and individual reflections. Program topics include goal setting, forming personal action plans, stroke risk factors and symptoms, self health monitoring skills, understanding medication, healthy eating, and regular exercise.
Goal / Mission
The goal of this program is to increase knowledge of stroke, encourage self-monitoring, and maintain healthy lifestyle changes to prevent secondary stroke.
Results / Accomplishments
When compared to a control group, participants in the intervention group had significant positive changes in knowledge of stroke warning signs (p < 0.001), treatment seeking response in case of a stroke (p < 0.001), medication compliance (p < 0.001), self blood pressure monitoring (p < 0.001), and dietary modification to reduce salt intake (p = 0.004). No significant improvement was found in exercise participation in the intervention group, but the control group had a significant decrease in exercise in the two months of follow-up.
